Instructions for Filling out Forms

Please view the materials below for help in completing the Probate and Guardianship Smart Forms.

Important note: You must complete steps 1 through 5 and create the PDF document before filing. Do NOT print the web form and attempt to file it. If you have questions, please refer to the contact information below. Thank you.

1. Click on desired web form

2. Fill out the web form.

If you need to come back to the web form later, click Save Draft on the bottom of the web form to save a web link in a location of your choice. Please remember to name it appropriately. Currently, pressing 'Create PDF' before using the 'Save Draft' feature will prevent you from saving your work. Please ensure that you save draft BEFORE pressing Create PDF.

3. Review your answers and click Create PDF button

4. You will be provided a link to download a PDF.

If you determine there is an error in your form after you have generated the PDF, close the PDF and return to web form. You can make the corrections and return to step 3 to create your PDF.

5. Save the PDF generated form to your computer and use this form to submit to the Florida Courts ePortal.

Please Remember

If you are filing a pleading that has an associated smart form listed below, you must file the smart form.
The Clerk’s Office will reject the filing if the smart form is not attached.
Make sure to use the correct case number formatting on the smart form.
When entering numeric values, please use whole numbers without decimals.
When inputting numerical values in the Initial Guardianship Inventory, Annual Accounting, Simplified Annual Accounting and Estate Inventory, please ROUND figures to the nearest dollar using a consistent method. i.e. .00-.49, round down; .50-.99, round up.
If you have supporting documentation, you will need to merge all PDFs into one file for submission to the portal.
