Probate and Guardianship Smart Forms
Instructions for Filling out Forms
Please view the materials below for help in completing the Probate and Guardianship Smart Forms.
Important note: You must complete steps 1 through 6 and create the PDF document before filing.
1. Click on the applicable Smart Form:
**Only the listed forms are recommended by the Court. If the specific topic is not listed here, a Smart Form should not be used. The Smart Forms should only be used for the listed type and cannot be accomodated for any other purpose.
2. Fill out the Smart Form:
If you need to come back to the Smart Form later, click "Save Draft" on the bottom of the Smart Form to save a web link in a location of your choice on your local drive. Be sure to save your work often through this "Save Draft" method. Please remember to name it appropriately so that it can be located later. This is the only way the Smart Form can be retrieved.
3. Click on the "Create PDF" button.
Make sure you review your answers before clicking on the "Create PDF" button.
4. You will be provided a link to download a PDF. This notification should appear on the lower left hand corner of your browser.
Do not use the print screen option, the system generates a barcode for each finalized PDF file. If there is an error in your Smart Form after you have generated the PDF, close the PDF and return to "Save Draft" version of the Smart Form. You can make the corrections and return to step 3 to create your PDF.
5. Save the completed PDF:
Save the downloaded PDF form to the local drive on your computer and make note of where the SmartForm is being saved. Use the pdf form that you just saved, along with any attachments that may be required, for submission to the Florida Courts ePortal through their website: www.myflcourtaccess.com .
6. Merging Smart Form with other documents (if any):
Please review this tutorial for instructions on merging additional documents.
If you are filing a pleading that has an associated Smart Form listed below, you must file the Smart Form
The Clerk’s Office will reject the filing if the Smart Form is not attached.
Make sure to use the correct case number formatting on the Smart Form.correct : PRC010001234 incorrect P-RC-01-0001234.
When entering numeric values, please use whole numbers without decimals or symbols.
When inputting numerical values in the Initial Guardianship Inventory, Annual Accounting, Simplified Annual Accounting and Estate Inventory, please ROUND figures to the nearest dollar using a consistent method. i.e. .00-.49, round down; .50-.99, round up. For Example: $2.49 should be entered as 2 (only the number(s)).
If you recieve the error message: (Line 8 should equal Line 14) or 14. Total Assets/Liabilities at Close of Accounting Period: - Missing or incorrect value. Please make sure 1) the value is rounded to the nearest dollar or 2) An accurate value has been supplied in section(s) 6 and/or 9.
